Athletics Forms and Links

Listed below are the MCDS athletic participation forms. These forms have been separated by Middle School and High School levels for your convenience.  In order for your child to participate in MCDS Athletics, these forms must be completed and returned to Mr. Clements before your child is allowed to participate in any physical capacity at practice.  There is also a new state law which requires student-athletes and their guardians to sign a concussion agreement before each individual sport season.  This means that if your child participates in a fall, winter and spring sport, a form must be signed for each individual sport season.

For Freshmen and Juniors

• Physical Evaluation History Form
For Sophomores and Seniors

A physical examination must be recorded once every two years for an athlete’s participation in athletics. As all Freshmen are asked to have a physical examination before sport participation, this form will be kept on file through their Freshman and Sophomore years.  For their Sophomore year, athletes will only need to turn in their Alternate Year Permit Card as a full physical examination is only necessary once every two years.

Concussion Information For Everyone

Wis. State Statute:  At the beginning of a season for a youth athletic activity, the person operating the youth athletic activity shall distribute a concussion and head injury information sheet to each person who will be coaching that youth athletic activity and to each person who wishes to participate in that youth athletic activity. No person may participate in a youth athletic activity unless the person returns the information sheet signed by the person and, if he or she is under the age of 19, by his or her parent or guardian.
